AI claims audit software for health insurers: a 2026 comparison

No single AI claims audit tool is best for every health insurer. Pick one that audits on the payer side, cites the evidence behind each finding, runs rules you can trace, fits your market's billing rules and leaves every decision to a human auditor. Below, seven options compared on what each one publishes.

Disclosure: Arkangel AI writes this page and is one of the options. Other vendors are described only with what their own websites say. If something is out of date, tell us and we will correct it.

How to choose AI claims audit software

  • Confirm it is built for the payer (auditing what you receive), not only for the provider preparing its bill.
  • Ask that every clinical finding cite the medical record and the guideline or policy behind it.
  • Check that the rules are written down, traceable and return the same result on the same batch twice.
  • Make sure it follows your market's rules: payer policies and coding in the U.S., the Manual Único de Glosas and its deadlines in Colombia.
  • Require a human auditor to decide every finding, with uncertain results kept as pending, never as automatic denials.
  • Compare vendors in a pilot on the same batch of claims, measured against your own audit team.

Evaluation matrix

Ask every vendor the same questions and ask for evidence, not promises.

Questions and evidence to request from every vendor
CriterionWhat to askEvidence to request
Clinical evidenceDoes each medical-necessity finding cite the record and the guideline or criterion behind it?Three findings from your own batch with the cited support.
Rules and traceabilityWhich rules run, who configures them and what data triggered each one? Does the same batch give the same result twice? The rule list with its reasons and a second run of the same batch.
Human reviewWho makes the final decision? What happens to what the system cannot confirm? Does anything reach the provider without approval? The approval flow and a log of who decided each finding.
Market rulesWhich payment policies, coding systems or national manuals does it apply, and how are they updated?Examples of findings with the rule or code cited.
IntegrationWhich files does it read (claims, invoices, medical records, contracts) and how does it connect to your claims system?A test with your real files, not a demo dataset.
SecurityWhich certifications does it hold and where can they be verified? How is protected health information handled?A current certificate or a public trust center, and the contract terms for health data.
Delivery modelIs it software your team runs, a managed review service, or both?Who does the work in each step of the pilot, in writing.

Arkangel AI

Complete audit of medical claims and accounts based on scientific evidence, with human review. 98 deterministic rules in three layers (27 administrative, 29 clinical, 42 financial). Uncertain results stay “not verified”; nothing is denied automatically and nothing reaches the provider without approval. ISO 27001:2022 certified; HIPAA compliance and a BAA on Enterprise plans.

Cotiviti

Presents “integrated prospective and retrospective payment integrity solutions” and says it employs “artificial intelligence and deterministic rules within a fully managed service model”. Lists coding validation, clinical chart validation, coordination of benefits and FWA pattern review.

Optum

“Payment integrity solutions for health plans” described as “one platform” across the claim lifecycle: cost avoidance before submission, pricing, editing, fraud detection and claim review before payment, and recovery after payment. Says “machine learning sharpens precision over time”.

Machinify

“Healthcare Payment Integrity Solutions” on an “AI operating system” that “brings together claims, medical records, contracts, and policies in real time”, with intelligent agents that improve “from humans in the loop”. Product lines: Coordinate, Subrogate, Audit, Pay and Pharmacy.

Alaffia Health

“AI built for health plans”: AI agents that “ingest thousands of pages of medical records”, “clinically tailored, human validated”, with “traceable source citations”. Offered as AI agents for your team or as managed services, across payment integrity, utilization management and appeals.

Codoxo

A “generative AI-driven Unified Cost Containment Platform” for provider education, payment integrity and special investigation units. It “integrates prepayment and postpayment processes” and adds pre-claim provider education so claims are submitted correctly the first time.

WIIT

“Plataforma inteligente de preauditoría y auditoría de cuentas médicas” for Colombian EPS, ARL, complementary plans and prepaid medicine. Says it crosses 14 data sources (including RIPS, FEV and MIPRES), applies more than 150 rules coded to the Manual Único de Glosas and uses AI to analyze clinical records.

Most vendors above describe U.S. health plans and programs. For Colombian EPS, the Spanish comparison covers more local options on both the payer and the provider side.

What changes in Latin America

U.S. payment integrity tools are built around U.S. coding, payment policies and programs. Health insurers in Latin America audit against national rules instead, so check that a vendor reads the files and codes of your market.

In Colombia, objections to a bill (glosas) must use the codes of the Manual Único de Devoluciones, Glosas y Respuestas (Resolución 2284 de 2023). Ley 1438 de 2011, article 57, gives the payer 20 business days to raise a glosa, the provider 15 to answer and the payer 10 to decide. In Peru, SBS Resolución 3202-2013 gives insurers 30 days to approve or reject a claim once the documentation is complete.

Payer or provider: not the same software

Provider pre-audit

The hospital or clinic checks its own claims before submitting them, to avoid denials and objections. Many coding and revenue-cycle tools work on this side.

Payer audit

The insurer audits what it receives: medical necessity, tariffs, supporting documents and duplicates, and decides what to pay. Every option in the table above describes work on this side.

Why it matters

The criteria look alike, but the workflow, the owners and the integrations differ. Ask each vendor which side its product is designed for.

How to compare vendors without bias

  1. 1

    Same batch for everyone

Prepare a de-identified batch with easy claims, hard claims and objections your team already knows.

  2. 2

    Same questions

Use the matrix on this page with every vendor.

  3. 3

    Your audit as the reference

Measure agreement, new findings with support and review time against your own team.

  4. 4

    Scope in writing

Ask for inputs, outputs, responsibilities and exclusions before going to production.

AI claims audit software FAQ

What is AI claims audit software?

It is software that reviews the claims a provider sends to a health insurer: supporting documents, tariffs, coding, medical necessity and duplicates. It proposes findings with their reason and evidence so an auditor can decide what to pay, what to object to and what to clarify, instead of reviewing every page by hand.

What is the best AI medical claims audit software for payers?

It depends on your market and workflow. U.S. health plans often compare Cotiviti, Optum, Machinify, Alaffia Health and Codoxo; Colombian EPS look at Arkangel AI and WIIT, among others. Compare evidence per finding, traceable rules, human review and fit with your billing rules, then decide with a pilot on the same batch.

What is the difference between prepay and postpay review?

Prepay review checks a claim before it is paid, so errors are prevented instead of recovered. Postpay review audits claims already paid and recovers overpayments. Many vendors offer both. In either case, ask how each finding is supported and who approves it before anything reaches the provider.

Can AI deny or object to claims on its own?

It should not. Every objection must be defensible in an appeal or reconciliation, with its reason and evidence. The safer design is that AI proposes findings, a human auditor decides, and anything the system cannot confirm stays pending. Arkangel AI marks those results “not verified” and never denies a payment automatically.

What should I ask a vendor about security and health data?

Ask which certifications it holds and where you can verify them, and how protected health information is handled and under which contract. Arkangel AI is ISO 27001:2022 certified, with HIPAA compliance and a BAA on Enterprise plans, 70+ security controls, penetration testing and continuous monitoring via Vanta, documented in its Trust Center.

Does Arkangel AI work outside Latin America?

Arkangel AI audits medical claims for insurers, EPS and IAFAS in Colombia, Mexico and Peru. U.S. teams can use it on Enterprise plans with HIPAA compliance and a BAA, and in the EU it is available under specific conditions with an Enterprise license. Contact the team to confirm the scope for your market.
